exception/unable to login as root with new 3.1.2 installation

All values seem to be correct, now the error message I get states that the
values are correct but I haven’t schedule the event but the event have been
schedule.
"
Passed XML Validation...
Passed Study Check...
Passed OID Metadata Check...
Passed Event CRF Status Check...
An error was thrown while reviewing validation errors. It could be that one
of several event CRFs is not scheduled. If you are trying to import data for
a repeating event, please make sure all occurrences of the repeat have been
scheduled.

Dear Ricardo,
Some points mentioned on
http://en.wikibooks.org/wiki/OpenClinica_User_Manual/ImportingData
Are the OIDs correct for the study, events, forms (you may need to specify a
particular form version) and subjects?
I found I had to schedule the Event for the subject before I could import
data into the event?
Are the values valid?
Yours,
Michael

The error message is:
"
Your XML is not well-formed, and does not comply with the ODM 1.3 Schema.
Please check it, and try again. It returned the message: The processing
instruction target matching "[xX][mM][lL]" is not allowed.

Dear Ira,
Your latest configuration sounds good (later versions of Tomcat and
PostgreSQL don't work, this is due to be fixed in 3.1.4).
Are you sure you have the datainfo.properties file set up correctly, and did
the db deploy correctly? Rather than spend a long time on this issue, I'd
be tempted to try redeploying OpenClinica from the war file - to a fresh
database and update the datainfo.properties file appropriately.
Yours,
Michael

I also tried various other versions of JDK including latest, different OS's

All successfully completed and got to the OpenClinica login window. Trying
