OpenClinica 3.1.2 issue when creating a discrepancy/annotation note, it places 3 copies of the note

I haven’t seen any reports in the issuetracker, so I’m not sure whether others have experienced this. When creating a discrepancy/annotation note, it places 3 copies of the note in the CRF. Only happens on second addition.
