Date Format Error Message Appearing for Non-Date Field Sections

Hi Everyone,
The instance of OpenClinca I'm running is experiencing a serious issue.
- When a section that does not have any date fields is completed, OpenClinica complains that the date is not valid:
The input you provided is not a valid date in DD-MMM-YYYY format.
- When a section with a date field is completed, OpenClinica still complains that the date is not valid:
The input you provided is not a valid date in DD-MMM-YYYY format. 24-Aug-2012
I'm running: Version 3.1.2-Community
This is on a live study.
Subject#1
I try to enter NO and the error message says that date is invalid?!

Subject#1
Again, the same error message?!

    Hi Jonathan,
It seems like your issue may be related to https://issuetracker.openclinica.com/view.php?id=10352 or https://issuetracker.openclinica.com/view.php?id=10353. May I ask what your browser locale is? If it is not en_US, can you try doing the same actions with a en_US browser locale and let us know the results?
    Thanks,
    Shaun

    Hi Jonathan,
The only other thing I can think of is that we have seen some weird date issues if the server running OpenClinica is Windows and was not installed with English\US as the default locale. Is your OpenClinica installation running on Windows? What is the locale Windows was installed with?
    Thanks,
    Shaun

    Hi Jonathan,
Alright since it is on Linux it is definitely not the issue with the locale like I thought as that is Windows only. I am sorry but I am personally out of ideas as to why that error may occur for you.
    Thanks,
    Shaun

    Hi Shaun,
We're running OpenClinica on Linux, specifically Ubuntu 10.04.4 LTS using the standard installation procedure.
We've been running the machine for more than 4 months now with english_US as the default locale.
Even if we're not entering a date and inputting a radio button response only, it still spits out the date error. It occurs with all of the subjects we've been attempting to update CRFs for.
