    A: should not happen but might occure due to a bug

    Q: and vice versa
    A: that's totally impossible in OC

    A couple questions that might help figure this out:
    - Look at the 'CRF Version integrity check' on the View CRF page. Do you
    see any problems reported there?
    - Are you missing entire subjects or just certain data items or event CRFs
    in those subjects.
    - Did you upgrade from an older version of OpenClinica, and are the
    missing data from prior to the upgrade? If so please identify what
    version(s) you used prior to 3.1.3.1

    Hi Cal,

    - The integrity checks of all the items in all used crf-s returns green "OK"s.
    - Three entire subjects (out of ca. 150) are missing. Previously, other minor data items have been missing as well (e.g. one or two uncompleted events), but these have since reappeared.
    - And yes, we have upgraded from older versions, but I will need to investigate the details.
